Report a Spill

FILLING OUT THIS FORM WILL NOT INITIATE AN EMERGENCY RESPONSE

Spillers are required (by Ohio law) to report their spill within 30 minutes (to the LEPC) then provide a comprehensive environmental clean-up report within 30 days.

Small Spills: Please complete this form to comply with the 30 minute reporting requirement. If you need your fire department to respond, call 911.

Large Spills: Contact EMA at (330) 722-9240 during business hours or (330) 725-6631 after hours. Then complete the form as soon as possible. This form will satisfy the 30 minute reporting requirement. If you need your fire department to respond, call 911.

If the general public needs to report a chemical spill, please call 911 to activate your local fire department.
